Player Registration Packet and other Documents

All players wanting to play in the 2006 2007 season need to fill out the registration packet. Players filling out the registration packet at the meeting need to download the online version and fill out page 5 “Informed Consent Form.” This page was not included in the packet handed out to you. Dont forget about signing up for a USLacrosse membership. The online form can be found here. Turn in a copy of your membership card or online receipt or if neither are available then turn in your USLacrosse number. Also, a copy of your class schedule printed from Owl Express needs to be turned in as well. All the paperwork needs to be turned in to an officer or coach by the end of practice on September 14th.
